New Patient Information
Welcome to West Michigan Family Medicine. We are glad you have chosen us for your family’s health care needs. As family physicians we care for men, women and children of all ages. We provide care for you when you are sick and work hard to keep you healthy. Services include childhood checkups and immunizations, physical exams and pap smears, mole removal, wart treatment, colposcopy, prenatal care and delivery of your newborn. If a referral is needed for further evaluation and treatment, we work closely with specialists that we know and trust.
Open Access Care - On any given day, we have room in the schedule for same day appointments. It is very important to us that we are able to provide same day care for injured or sick patients. This allows us to provide better care because you are usually able to see your own doctor for urgent healthcare needs. If you need a same day appointment we encourage you to call early in the day.
Financial Policy - Please bring your insurance information to each visit. Payment is expected at the time of service unless prior arrangements have been made. There is a $10 statement fee for any co-pay that is not paid at the time of service. You may pay with cash, check, money order, Visa or Mastercard. Health insurance is a contract between you and your carrier. It is important that you understand the coverage that it provides. WMFM participates with most health plans and will courtesy bill all others. If you do not have any insurance coverage, we offer payment plans for established patients. We also offer a 25% discount if services are paid in full at the time of service with cash.
Twenty-Four Hour Coverage - A doctor is available twenty-four hours a day, every day of the year. We encourage you to address routine questions during office hours. We also utilize an answering service for after-hours emergencies. If this service is used excessively or inappropriately a $15 charge per call will be assessed.
No Show Policy - We require 24 hours notice for appointment cancellation. Timely cancellations allow us to accommodate other patients’ needs. If there are three “no shows” the patient and family will be discharged from the practice.
